Kohen 

פבפ stands for the title & name, in Seder Chazzanut noted as NN, see Names

כהן קרב וכהן, יעמוד השם הטוב פבפ
לקרא בתורה, וישמרהו צורו

Levi and Yisrael

יעמוד השם הטוב פבפ
לקרא בתורה, וישמרהו צורו

One before last, on Shabbat usually sixth, on Yom Tov fourths, called “Samuch”, 

יעמוד השם הטב פבפ
לקרא בתורה, וישמרהו צורו, סמוך

Lasts, called “Mashlim”

יעמוד השם הטוב פבפ
לקרא בתורה, וישמרהו צורו, משלים

Maftir. 

On rare occasions Maftir is called with name. He then is called like the 7^th, but with the addition מפטיר in stead of משלים

Exceptions

When there is no Levi, the Kohen is called a second time. He skips the ה’ עמכם

קרא כהן במקום לוי

When there is no Kohen, a Yisrael is called as first. A Levi is not called.
The section of the Levi is given to another Yisrael.

יעמוד השם הטוב פבפ
במקום כהן, שאין לנו כהן
לקרא בתורה, וישמרהו צורו

A Kohen or Levy may be called as Mashlim (7^th)

יעמוד השם הטוב פבפ
אף על פי שהוא כהן / אף על פי שהוא לוי
וישמרהו צורו, משלים

When the Chazzan calls himself.

He adds סמוך or משלים if he is 6^th or 7^th. He skips the ה’ עמכם and the congregation says the אמן after the Beracha.

אני הקרא אקרא (סמוך / משלים)

When the Chazzan calls himself. 

He skips the ה’ עמכם and the congregation says the אמן after the beracha.

אני הקרא אפטיר

When the Chazzan is a Kohen or Levi and calls himself 1^st or 2^nd
The Kohen adds במקום לוי if there is no other Levi

He skips the ה’ עמכם and the congregation says the אמן after the Beracha.
